Notice of Infringement – Claim

- Contact information of the complaining party;
- A statement of good faith belief of unauthorized use;
- A statement that the notification is accurate and the notifier is authorized by the copyright owner.

Counter Notification – Restoration of Material

- Description of the removed material and its original location;
